To Receive Tax Forms

The best, cheapest and easiest method to receive paper copies of tax forms and instructions is to call the IRS and have them mailed directly to your home. The number is 1-800-TAX-FORM (1-800-829-3676). 

The library has limited paper copies of tax forms available, and some tax forms are available for copying. Copies cost 15 cents per page.

The following federal tax forms are available on the lower level at the Bloomington Public Library:

  • 1040 Instructions
  • 1040 Tax Return form
  • 1040 A Instructions
  • 1040 EZ Instructions
  • Schedule A
  • Schedule B
  • Schedule L
  • Schedule M

If you need to print your tax forms at the library, the Adult Services department has Internet computers available on a walk up/first-come, first-served basis. Printing costs 10 cents per page.

Librarians cannot legally assist you with filling out your tax forms.

Free Tax Assistance is available in the Bloomington-Normal area.

Circuit Breakers

The Illinois Department on Aging

  1. administers the Circuit Breaker program, which provides grants to senior citizens and persons with disabilities and
  2. enrolls qualifying individuals in the Illinois Cares Rx program (formerly Circuit Breaker Pharmaceutical Assistance and SeniorCare), which provides “wrap around” benefits with the new Medicare Part D prescription drug program.

LIFE Center for Independent Living
There are five (5) scheduled workshops to teach people how to fill out the Circuit Breaker forms:

  • Friday, February 5 from 2:00 PM to 4:00 PM
  • Friday, February 19 from 5:30 PM to 7:30 PM
  • Friday, March 5 from 2:00 PM to 4:00 PM
  • Friday, March 19 from 5:30 PM to 7:30 PM
  • Friday, April 9 from 2:00 PM to 4:00 PM
